Centennial Family Eyecare – Insurance Assignment Policy

We’re happy to help you understand and use your insurance benefits. Our team will do our best to verify your coverage and submit claims for services and materials on your behalf. Please keep in mind that benefit information provided by your insurance company is not a guarantee of payment.


To ensure accurate billing, all current insurance information must be provided at the time of your visit, including details for any Worker’s Compensation or Personal Injury claims. Patients are responsible for any copays, coinsurance, deductibles, and non-covered services at the time services are provided.


Services That May Not Be Covered

If your insurance company does not cover a service or material for any reason, the balance becomes the patient’s responsibility. You may choose to pursue reimbursement directly with your insurance provider if applicable.


Coverage varies by plan, but the following items are commonly not covered by insurance:

  • Specialized diagnostic testing, such as routine retinal photography

  • Eyeglass lens and frame options (UV protection, anti-reflective coating, scratch resistance, etc.)

  • Contact lens evaluations, fittings, follow-up care, and solutions

  • Premium, thinner, oversized, or specialty lenses

  • Elective Dry Eye and Anti Aging Eye Procedures

Referrals & Pre-Authorizations

Some insurance plans require referrals or prior authorization. While we’re glad to assist with submitting claims, we’re unable to assume responsibility for services that have not been authorized. To avoid unexpected charges, please make sure any required referral or pre-authorization is in place before your appointment.


Unpaid Balances

We understand that billing questions can arise and are always happy to help. Statements will be sent for any remaining balances. If a balance remains unpaid after the initial billing period, it may be referred to a collections agency and could be subject to additional fees.
